Understanding Your PHQ-4 Screening Results
The Patient Health Questionnaire-4 (PHQ-4) is an ultra-brief, validated screening tool consisting of a 2-item anxiety scale (GAD-2) and a 2-item depression scale (PHQ-2). Here is how psychologists interpret cumulative scores:
0 – 2 Points
Normal / Minimal
Suggests healthy emotional balance. Maintain active rest, quality sleep, and regular mindfulness habits.
3 – 5 Points
Mild Psychological Strain
Noticeable tension or brief blues. Recommended to re-evaluate boundaries, exercise, and consider a wellness check-in.
6 – 8 Points
Moderate Symptoms
Symptoms begin to interfere with daily focus and relationships. Consultation with a licensed counselor is strongly advised.
9 – 12 Points
Severe Emotional Distress
Significant psychological burden. Prioritize booking an immediate clinical consultation or contacting our crisis support desk.
